On June 4th, 2025, at 9:04 Caernarvon Fire Company (Churchtown), Twin Valley Fire Department, Elverson-Honey Brook EMS and Wellspan Medics were dispatched to the intersection of N. Red School Road and Route 23 for an auto accident.

Caernarvon Fire Company units along with Elverson-Honey Brook EMS made the response and were advised by the Lancaster County 911 Center of a dump truck versus a vehicle with at least 2 patients. Additional callers reported 1 subject was trapped with multiple people injured. The Lancaster County 911 Center upgraded the call to an accident with entrapment.

Caernarvon Fire Company units arrived to confirm a 2-vehicle accident with multiple patients and 1 subject was confirmed trapped. Caernarvon Fire Company personnel setup the command and requested additional Medic's and Ambulances to the scene. Ambulance 189 arrived and began triaging patients at the scene. Caernarvon Fire Company personnel began extricating the patient from the vehicle for EMS. As additional EMS units arrived, they were given their assignment and patient(s) by the command.

The commander reported the extrication of 1 subject was complete at 9:13 and advised the Lancaster County 911 Center to recall Twin Valley Fire Department (Rescue 69). Ambulance 189 started transport of 1 subject and requested the "Critical Care Truck" from Tower Direct be dispatched to meet them enroute to the area trauma center. All other EMS units packaged their respective patients and started transport to the appropriate facilities.

Caernarvon Fire Company units remained on the scene to assist the Pennsylvania State Police with their investigation. Fire police from neighboring agencies were requested to the scene to shut down the roadway due to an extensive investigation of the accident scene.

Ambulance 189 cleared the call at 11:53.
